Accepted Documents to Prove Spouse Scholarship Eligibility
You must prove you are a military spouse by providing a copy of your marriage certificate and one of the following documents from the applicable category below:
Active Duty Spouses (Air Force, Army, Marine Corps, Navy, NOAA, Public Health Service)
- Service member’s most recent copy of Leave and Earnings Statement (LES)
- Service member’s Social Security Number
National Guard or Reserve Spouses
- Service member’s most recent copy of Leave and Earnings Statement (LES)
- Service member’s current orders
Retiree Spouses
- Service member’s most recent copy of Retirement Pay Statement
- Service member’s Retirement Orders (DD214)
Spouses of the Wounded or Fallen
Document must verify service member’s injury or death in a combat zone while performing a mission in support of or during military training for the Global War on Terror (since September 11, 2001).
- Service member’s Certificate of Release or Discharge from Active Duty (DD214)
- Service member’s injury report
- Service member’s Purple Heart certificate
- Service member’s death certificate
- Service member’s Report of Casualty (DD1300)
